What Is A First Report of Incident?

A First Report of Incident captures and forwards the most critical elements of an incident as quickly as possible. For most groups, the key pieces of information are things like:

  • What happened?
  • How bad is it?
  • When did it happen?
  • Where did it happen?
  • Additional comments
  • Optional photos or attachments

Once a form is submitted, you and your managing group can receive instant email and/or push-notifications so you can immediately know what happened and begin taking the necessary actions.

This form is a sample report which can be imported into your KPA Flex account. All forms including this one can be easily modified to fit your organization's policies and procedures.
